FUSE Day Support is dedicated to providing high-quality day support services for adults with intellectual and developmental disabilities in Henry and surrounding counties.

OUR MISSION:

To improve the lives of individuals with intellectual and/or developmental disabilities through promoting compassionate care, independence, and purposeful engagement. Our mission is to create an environment in which individuals can feel supported and empowered to develop meaningful skills and relationships, and reach their full potential. FUSE is dedicated to the values of inclusion, community building, and advocacy work on behalf of our participants.

  • Our services are designed for adults with intellectual and/or developmental disability.

  • Our services are covered by the Developmental Disability (DD) Waiver. Individuals who have been assigned a waiver can choose to participate in our program without any cost out of pocket.

  • Usually, an individuals' Support Coordinator will submit a referral for our program. However, guardians, caregivers, group home staff, or other stakeholders can submit a referral form, contact us through our website or by phone/email to express interest. Our Program Director or Office Administrator will reach out to you to start the intake process. We will then schedule a time to meet for a shared planning meeting where we identify goals for the participant and make a plan for starting.

  • Our staff consists of a Program Director (Board Certified Behavior Analyst), Office Administrator, and multiple Direct Support Professionals (DSPs).

    We aim to keep our staff ratios low, striving for a 1:5 ratio (1 staff per 5 participants) to allow for a high quality of support.

    All staff are required to complete trainings in compliance with state and federal regulations, including behavior management, medication administration, CPR & First Aid, and other relevant trainings.

  • FUSE Day Support does not offer transportation to/from the program. There are many Medicaid-funded companies available in Henry and surrounding counties that can help with transportation for participants. You or your Support Coordinator can arrange this.
